• Permission for ID Print letter on company letterhead
• Portrait picture of the individual(s) on a white or light background
• First and Last Name
• Positions/Job Title
• ID Number (Optional)
• Authorizing Signature (Optional)
• Employee Signature (Optional)
• Expiration Date (Optional)
